Invisalign Mandibular Advancement vs. Twin Block: Which Is Better for Growing Smiles?

Saturday, August 16, 2025

Both aim to correct the bite by posturing the lower jaw forward, but they look, feel, and function very differently. If you’re wondering which one might be the right fit, here’s a breakdown.

What Is the Twin Block?

The twin block is a removable functional appliance made of two acrylic plates (upper and lower) with angled bite blocks. When worn, it positions the lower jaw forward, stimulating jaw growth and helping move the lower jaw forward to correct the bite over time.

Pros:

Effective for moderate to severe cases, more affordable than clear aligners.

Cons:

Bulky and can affect speech, mouth produces a lot of saliva when worn (this is temporary), visible when worn, success depends heavily on patient compliance (needs 20+ hours per day). initial phase of treatment with twin block takes around 6-9 months providing the patient wears the appliance as advised.

What are Aligners with Mandibular Advancement?

Aligners with Mandibular advancement use clear aligners fitted with small "precision wings" or barely visible blocks that gently guide the lower jaw forward. Unlike twin block, Invisalign can straighten teeth and advance the jaw at the same time.

Pros:

Nearly invisible, much more aesthetic, comfortable, combines jaw correction with tooth alignment in one phase.

Cons:

Higher cost, still compliance-dependent (must be worn 22 hours a day).

Side-by-Side Comparison

Twin Block - Noticeable, Bulky, affects speech, tooth straightening corrected after jaw correction, Compliance Removable, kids often skip wear and cost is lower.

Aligners with Mandibular advancement - Nearly invisible, Slimmer, easier to adapt to, tooth straightening corrected at the same time as mandibular advancement, easier to wear but still must be worn and cost is higher.

So, Which Is Better?

There’s no one-size-fits-all answer.

Twin Block may be the best choice for patients with significant jaw discrepancies and families looking for a more cost-effective solution if patient qualifies for NHS orthodontic treatment, twin blocks are also available on a private basis but are only effective when the patient is growing.

Aligners with Mandibular advancement may be ideal for patients who want a more aesthetic, comfortable option and are motivated to wear aligners consistently.

Ultimately, the right appliance depends on the patient’s age, growth potential, severity of the bite problem, and lifestyle. An orthodontist can help decide which path gives the best chance for a healthy bite and a confident smile, however each parent knows their child as a person and which brace option would suit their child best as an individual.

Bottom line: Both Invisalign MA and twin block can successfully advance the lower jaw during growth. It’s really a matter of matching the right tool to the right patient.
